This form begins the booking process for a school field trip to see a performance at Chicago Children's Theatre.
The student performance group minimum is ten people.
Chicago Public School tickets are $13 for students and adults. Private School tickets are $15 for students and adults. Scholarship tickets at reduced prices may be available.

Group Leader represents that they have the right, capacity, and authority to enter into this Agreement and that Group's participation and performance and the rights the Group Leader has granted in this Agreement will not conflict with or violate any commitment or understanding Group Leader has with the Group, as well as any other person or entity.

Accessibility Needs*
We strive to be inclusive of all populations both on and off the stage. Please review the below for any accessibility needs which we can accommodate. If you have needs not listed, please indicate what would be useful in the notes field below. Please note that some requests can require up to 4 weeks to fulfill.
- Assistive Listening Devices (guests who are hard of hearing can request devices that will help amplify the sound of the play).
- Audio Described Performance (for guests who are blind or have low vision, an Audio Describer provides in-depth descriptions of performer characterizations, costumes, and staging).
- ASL Interpretation (all dialogue and lyrics are interpreted by a licensed American Sign Language interpreter during the show).
- Open Captions (Open captions will be displayed on a large screen for audience members who benefit from reading along with the action).
- Sensory Friendly Performance (Tailored specifically for guests on the autism spectrum or with other sensory sensitivities, we offer performances with sensory modifications to production elements such as lighting, sound, and volume).

Lunch Needs*
CCT allows groups to arrive early or stay after a performance to enjoy their lunches at no additional cost to the school. We are also located two blocks from Skinner Park, if you and your students want to bring your lunch with you.

Cancellation Policy
Cancellation Policy: Cancellations may be made without penalty up to 6 weeks prior to the performance date. Groups cancelling their reservation within 6 weeks or less of their scheduled performance date will be charged 50% of the total balance due to the credit card provided on this form.

Cancellation Policy*
I understand that by completing this form I acknowledge that I am responsible for the payment of any seats that are later agreed upon once final agreements are completed. I understand that cancellations must be made by email at least 6 weeks prior to my scheduled performance date. Any cancellations made within the 6 week deadline will be subject to a 50% cancellation penalty. Completion of this form confirms that you understand and agree to the full terms and conditions of the cancellation policies, which can be found at the bottom of our website.
